Privacy Policy
What we collect, why, who we share it with, and the choices you have.
Who we are
This policy covers the Continuis website, the Continuis assistant and the Continuis patient portal, operated by Continuis Health, Inc. ("Continuis", "we", "us"). Medical care is provided by independent licensed clinicians through affiliated medical practices.
What we collect
- Account details: name, email address, phone number, date of birth, sex at birth, home address and preferred language.
- Health information you give us: what you tell the Continuis assistant, your answers during a check-in, medications, allergies, conditions, and anything you share with a clinician.
- Care records: visit notes, diagnoses, care plans, referrals, work or school notes, and your preferred pharmacy.
- Payment details: handled by our payment processor. We see the last four digits and the payment status, never your full card number.
- Technical information: device and browser type, IP address (used for security and abuse prevention), and the approximate state you're in, which you can confirm or change.
How we use it
- To provide care: to prepare a summary for your clinician, run video visits, and deliver your notes, prescriptions and care plans.
- To run your account: sign-in, security, support and billing.
- To keep the service safe: detecting fraud, abuse and automated sign-ups.
- To meet legal duties, such as medical record retention and responding to lawful requests.
- To improve Continuis, using information stripped of details that identify you, or with your permission.
What we never do
- Sell your personal information or health information.
- Use your health information for advertising or share it with advertisers.
- Put health details in page addresses, page titles or analytics events.
Who we share it with
- Your clinicians, to treat you.
- Service providers that help us run Continuis, under contracts (including HIPAA business associate agreements where required) that limit their use of your information:
- cloud hosting and sign-in (Amazon Web Services);
- AI model services that power the assistant;
- video visits (Zoom);
- payments (Stripe);
- bot protection (Cloudflare);
- Google, if you choose to sign in with Google.
- Pharmacies and labs you choose, to fill prescriptions or orders.
- Others when the law requires it, such as public health reporting or a valid court order.
Cookies and local storage
We don't use advertising or cross-site tracking cookies. Your browser stores a few small items so the service works:
- a sign-in handle, so you stay signed in;
- an anonymous check-in token, so you can finish a check-in before creating an account;
- a random device identifier used for security.
None of these contains health information.
Your choices and rights
You can view and update your details in your portal, choose what you share in a check-in, and delete a check-in you started before creating an account. Depending on where you live, you may also have the right to:
- access your information;
- correct it;
- delete it;
- get a copy of it;
- withdraw consent.
Email [email protected] to make a request. We'll verify it's you and respond within the time the law requires. Some medical records must be kept for a set period even if you ask us to delete them.
Consumer health data (Washington, Nevada, Connecticut and similar laws)
Consumer health data we collect includes the health information listed above. We collect it to provide the care and services you ask for, and we share it only as described in this policy. You can request access to it or deletion of it, or withdraw consent, at [email protected]. We don't sell consumer health data.
California residents
We don't sell or "share" personal information for cross-context behavioral advertising. California residents may exercise the rights to know, delete, correct and limit use of sensitive personal information by emailing [email protected]. We won't discriminate against you for exercising these rights.
Security
Information is encrypted in transit and at rest. Access by our staff is limited to those who need it and is logged. Clinician accounts require two-factor sign-in. No system is perfectly secure, so please tell us at [email protected] if you think your account has been misused.
Children
Continuis is for adults 18 and over. Care for minors requires a parent or guardian and isn't available yet.
Changes
If we change this policy in a way that matters, we'll update the date above and let you know before the change applies to you.
